To work with a caseload of individuals, working and supporting individuals in their own homes. The role involves working within a team but you will be essentially lone working. Interagency working will be required to ensure a holistic response to an individual’s support needs.

 

To provide intense outreach support to individuals who have issues such as failing to sustain accommodation, substance misuse, mental ill-health, and who may present with challenging behaviour. The aim is to support individuals to stabilise their situation, assisting them to maintain their tenancy and address their substance misuse issues.

 

Staff will also be involved in an out of hour’s on-call rolling rota.

 

Candidates must have prior experience in supporting individuals with complex needs and lone working.
